Livorno is the gateway port for two of Italy's most-visited cities, which makes it the Mediterranean's classic decision port: Florence is 90 minutes away, Pisa 30, and you can realistically do only one well. Getting ashore
Cruise ships dock inside a working container port, so you cannot walk out - the port shuttle (about €5-7) runs to Piazza Grande in Livorno town, from where the station is a short bus or taxi ride. Trains to Florence's Santa Maria Novella station cost about €10-12 each way and take 80-90 minutes; Pisa is 15-20 minutes and €3-4. Cruise-line and independent "Florence on your own" shuttles (€25-45 return) go direct from the ship to the center.
What to do in 6-10 hours
Choose Florence for the full Renaissance day: the Uffizi or Accademia (both €16-25, book ahead), the Duomo exterior, Ponte Vecchio and a long lunch. Choose Pisa for the gentle half-day: the tower and cathedral square are compact and flat, leaving time for Livorno's canals and market back at the port. Doing both is possible only via a ship or private tour, and even then it is a photo-stop day, not a visit.
Walking level
Florence's center is flat and pedestrianized but big - 4-6 miles for a good day, on smooth stone paving that is kinder than Rome's cobblestones. Museum time means standing. Pisa's miracle square is small and flat - the easy option for limited-mobility days. Neither involves hills.
Excursion or independent
Florence independently by train is the best-value day in the Western Med: about €25 total transport versus €100-150 for the ship's bus-and-walk tour. The risk is the return train - leave Florence at least 3.5 hours before all-aboard, and know that Italian regional trains occasionally run late. A middle path many choose: the independent port-to-Florence shuttle, which keeps ship-day timing but drops you free in the center. The full trade-off is in ship excursion or independent.
